Some underlays will form a moisture barrier, help deal with the uneven surface (it won't be totally flat), as well as provide insulation to the concrete. Going to be a bitch to heat otherwise.

Fit it yourself. It's not hard to do, just take your time at the start. The first panels are the most important in getting alignment to walls (you room will no be square).

B&Q used to have a decent selection. We got our engineered wood from there. Lots of choice of material and style.
