Comment	Daft question - have you checked your online record? https://vacs.nhs.scot/csp If the details are there but the app isn't picking them up, that's one problem but at least you can print off the form. You should contact the helpline again with the location, dates, and times of your vaccinations (all on the letters you got) to help them trace you. Contact your MSP if you continue to have no progress.
