I have had two close relatives with Alzheimer's. It is awful to watch your loved ones deteriorate before you. They can say and do things that will break your heart. However bad it is for you, consider how bad it is for the Brother and Nephew. People want to care for their own as long as they can because they have real fears that institutions will neglect their loved ones should they go into care.

It is a difficult one, it must be really bad for you. I just don't see how an internal family discussion will have good outcomes with somebody from outside the family being involved. I think if you get involved you might end up really regretting it or feeling very guilty. You must already have a sense of this or you would not have posted your question.

Another point you should keep in mind is that while everybody is different, Alzheimer's has a typical progression and this sounds like the poor lady is mid to late stage.
In other words its not going to go on forever. This lady will not be able to be cared for at home (by only the brother) for much longer. That is inevitable.

In any case get some informed advice from one of the dedicated Alzheimer's support networks already mentioned before saying anything.
