Try to get into a new/different environment if at all possible, (and assuming you don't feel so shitty you can't move). Bump up whatever your using to replace the opiates, even if it feels like it will mess-up you timeline for coming off. Try to be around 'normal' people, and perhaps go as far as telling a close friend (if you still have any) that you're struggling and shouldn't be left alone for more than 30 mins. If you've done a week you're doing great - although it won't feel like it. But the transition from physical to mental withdrawal can be tough unless you actually have something to aim for.

Jumping off a sinking ship won't help you if you don't have a liferaft within swimming-distance.
