I think it's probably down to how quiet bikes are - we all realise this consciously, of course, but most of the time we're "looking out" subconsciously - it's often a "wtf" when I'm overtaken by a cyclist on a shared path, and nearly a close call a couple of times when I've moved out of the wrong way (there's not really any time to check whether stepping right/left is better once you can hear them).

I'd suggest cards taped to the forks hitting the spokes, or a speaker playing white noise, but then you'd probably get people complaining about that instead...
An excessively bright head torch might help make people pause before stepping out?
