I have seen them embedded in the close wall, the older 'electicity board' cables don't usually have pipework though the older cables are armoured, this is all supposition but if he was working on close lighting at the close mouth and it was a load explosion there is a likelihood of it being before the cablehead. The cablehead also has protection fuses to minimize issues on cables after it going into the close for isolation. Which also means It also could be he was trying to isolate the supply from the cablehead and it went bang. The board equipment is getting very old and the older 'ironclad' cableheads could be a bit dodgy.
