Yes, heat pumps generally need to be installed in energy efficient buildings in order to operate effectively - as you can see, this is a supplementary grant alongside the grants you can get for those efficiency improvements, so you can do both at the same time!

Yes, radiators will often need to be replaced, but not always - it's very property-specific. Pipes usually don't need to be upsized. Grants/loans can cover these things too.

Anyway, if you're happy with your existing system, and also not interested in getting away from burning fossil fuels (either on cost or carbon) then this isn't really being targeted at you. But bear in mind that at some point over this decade replacing your (natural) gas boiler won't be an available option.
