The whole thing.

A vocal channel will NORMALLY have a low cut to 150hz, depending on the vocalist, the stage, the mic etc.

The proximity effect raises low and mid low frequencies the closer you get to a microphone,

For example commonly cut my own vocal in the studio at 150hz, and low pass at 100hz~, and this means i can get right up on the mic and not have out of control low end.

By cupping a microphone it changes the pickup pattern from cardioid to omnidirectional, meaning it picks up everything around it rather than in a cardioid pattern, including all of the noise on stage.

The same happens with metal singers. With enough warning an engineer could EQ around it but it once again comes down to professionalism and time.
