I have seen something like this but at home. We kept milk (cows milk) in a button operated flask. We didn't clean it for a few months. The milk didn't come out right. So I opened it up and to my disgust there was old cheese like bits stuck in the mechanism. The milk was super hot and over time it grew in the flat, so the main vessel was cleaned and the mechanism with the spring just rinsed.

I wonder if the coffee shop you went to had the soy milk in a flask or some kind of heating device and not cleaned and something dislodged it?
