Used to work in pharmacy, in our one you were supposed to get privacy but it really depends on the shop and who the pharmacist for the day is and if they're going to actually respect that.
Also depends on if the patient cares or not and asks about it where others can hear.

Technically, to abide by patient-confidentiality, you as a general member of the public are not supposed to know what any other patient is there for, even if it's a prescription box of paracetamol. However, if either the pharmacist or patient don't care enough to be careful about how loud they're talking, everyone in hearing distance will know.
