Trans-friendly in this context, in my experience, usually means staff are appropriate and respectful towards patients in relation to them being trans. You'd be surprised how many are not sadly.

Op, I have been in this situation, if I were you I'd contact them and ask if they do this type of shared care. The NHS usually doesn't but it doesn't hurt to ask.
