If you are dealing with a genuinely ill-minded person don't discount the notion that she is actually a patient or service user and is pretending to be/ spinning stories about being a nurse.
The woman's whole life might be a sad fantasy.
I'd be looking into things further, not just for your own sake, but for hers too.
