Setting aside the fact that I sincerely doubt anyone is just casually hopping on the wait list for medical transition for a laugh, if you wanted efficiency you'd get rid of the whole "Separate clinics for Trans healthcare" and just let people go to their regular GP and get the same hormones that are routinely prescribed for any number of other conditions.
