It's an English thing. In Scotland your water use is unlimited and paid as part of your Council Tax (you'll see it itemised on your Council Tax bill).

In England it's based on usage. If you have a meter you'll be billed for what you use, but some older houses don't have a meter and instead get billed a flat rate based on the "Rated usage", basically an estimate of how much water a house that size should use. When it comes up you generally see people recommend getting a meter over the rated usage because most people will save money that way.
